When the Percussion Enters

Your body moves before you decide to move.

A car swerves into your lane and your hands tighten around the steering wheel.

Something crashes behind you and your shoulders rise.

Someone steps toward you aggressively and you find yourself leaning backward.

A person you love suddenly pulls away and, almost before you understand what is happening, you reach toward them.

Or perhaps you do the opposite.

You become completely still.

Later, you may have an explanation.

I was scared.

I wanted to get away.

I needed them to stay.

I froze.

But once again, the explanation is not necessarily where the sequence began.

In the previous pieces of this series, we listened for orienting and then for shock.

Something changed.

The conductor raised the baton.

The expected music stopped.

Now the orchestra has to respond.

And this is where the percussion enters.

From Detection to Action

A nervous system does not exist merely to understand the world.

It helps an organism do something in the world.

Move toward.

Move away.

Become still.

Protect.

Reach.

Call out.

Brace.

Escape.

Human beings can think about these responses afterward, but many defensive processes are coordinated rapidly and outside deliberate conscious control.

This is important because trauma is often remembered through the lens of hindsight.

We look backward with information we did not have at the time.

I should have left.

I should have screamed.

I should have known.

Why didn't I fight back?

But the nervous system was not sitting at a conference table calmly reviewing the available options.

It was responding.

Meet the Periaqueductal Gray

Deep within the midbrain is an area called the periaqueductal gray, usually shortened to PAG.

The PAG surrounds the cerebral aqueduct and participates in interconnected neural systems involved in defensive behavior, pain modulation, autonomic function, and other survival-relevant processes.

Animal and human neuroscience research has linked PAG circuitry to different defensive patterns depending on context and threat.

That makes the PAG particularly interesting for understanding how brains coordinate responses when something important happens.

But we need the same caution we have used throughout this series.

The PAG is not the brain's "trauma center."

Fear does not live there.

Freeze does not live in one tiny neurological box.

And a human trauma response cannot be reduced to a colorful brain diagram.

The PAG participates in broader networks involving cortical, limbic, hypothalamic, brainstem, sensory, motor, and autonomic systems.

Human defensive behavior emerges from systems.

Not isolated structures.

DBR draws on this neurobiology to develop a clinical model for working with defensive responses that may emerge after orienting and shock.

The Percussion Section Doesn't Ask Permission

Think about percussion in an orchestra.

A drumbeat can change the entire physical experience of a piece.

You don't merely hear it.

You feel it.

It creates momentum.

It signals movement.

Your nervous system's defensive responses can have a similar quality.

Your heart changes.

Your breathing changes.

Muscles prepare.

Your posture shifts.

Attention narrows.

You may feel an urge to run, push, hide, curl inward, reach for someone, or become completely still.

And only afterward does language catch up.

I'm afraid.

This is one reason the phrase fight or flight is useful but incomplete.

Human defensive responding is far more complex than choosing between two buttons labeled FIGHT and RUN.

Sometimes Defense Looks Like Stillness

Imagine hearing footsteps behind you while walking alone at night.

Your attention shifts.

Your body becomes alert.

Perhaps you walk faster.

That response makes intuitive sense.

Now imagine something very different.

Someone says something devastating to you.

And you don't move at all.

You stare.

Your body feels heavy.

Your voice disappears.

Later, you are furious with yourself.

Why didn't I say anything?

We tend to interpret action as strength and immobility as failure.

Biology does not make that moral distinction.

Defensive systems can involve active and passive patterns, and the response that emerges depends on many factors, including how the nervous system evaluates the situation and what options appear available.

Stillness can be part of defense.

Inhibition can be part of defense.

And neither tells us anything about someone's character.

The Body May Prepare for Something That Never Happens

There is another experience that can be difficult to understand.

Sometimes there is a powerful impulse to act, but the action never occurs.

You want to push someone away.

But you don't.

You want to run.

But you stay.

You want to scream.

Nothing comes out.

You desperately want to reach toward someone.

You stop yourself.

Yet something of the action may still be detectable in the body.

The shoulders.

Hands.

Jaw.

Throat.

Chest.

Legs.

Face.

These subtle action tendencies are interesting in trauma therapy because they remind us that an experience is not composed only of thoughts and emotions.

There are also preparations for movement.

The orchestra was getting ready to play something.

Even if the note never became audible.

Defense Isn't Always About Physical Danger

This becomes even more complicated when the perceived threat is relational.

Imagine your partner looking at you and saying:

"I don't know if I can do this anymore."

No weapon.

No approaching animal.

No physical attack.

And yet your entire body changes.

Your stomach drops.

Your chest tightens.

You lean forward.

Wait.

Maybe you start explaining.

Maybe you apologize for things you don't actually believe were wrong.

Maybe you become angry.

Maybe you leave before they can leave you.

Maybe you become strangely blank.

Human survival has always depended partly on relationships.

From infancy onward, other people can represent comfort, protection, belonging, resources, identity, and safety.

Consequently, threats involving connection can recruit powerful emotional and defensive responses.

The nervous system does not require a tiger before something can feel urgent.

Sometimes the threatened loss of another person is enough to bring in the drums.

When Two Sections Play Against Each Other

Now imagine something even more complicated.

The person you want comfort from is also the person who frightened you.

Part of you wants to move closer.

Another part wants distance.

Your body leans toward them while simultaneously becoming tense.

You want reassurance but don't trust the reassurance when it comes.

You are afraid they will leave and afraid of what happens if they stay.

This can feel incomprehensible.

Why am I doing this?

Why can't I just walk away?

Why do I miss someone who hurt me?

Why do I push people away and then panic when they leave?

From the perspective of an orchestra, however, we can imagine competing sections entering simultaneously.

One system is organizing around connection.

Another is organizing around defense.

The result is not a clean melody.

It is dissonance.

And that conflict will become important later in this series when we explore attachment and trauma.

The Problem With Calling Everything "Dysregulation"

In mental health language, we sometimes flatten enormously complicated experiences into one word:

dysregulated.

The person is crying?

Dysregulated.

Running away?

Dysregulated.

Shut down?

Dysregulated.

Clinging?

Dysregulated.

Angry?

Dysregulated.

The word can be useful.

It can also become so broad that it tells us almost nothing.

DBR invites a more specific curiosity.

What happened first?

What did attention orient toward?

Was there shock?

What defensive response began to organize?

Was there an urge toward movement?

An inhibition of movement?

A conflict between approaching and escaping?

Instead of simply saying the nervous system is "activated," we begin listening for individual instruments.

That creates a much richer understanding of the experience.

Why DBR Slows Things Down

When someone tells the story of a traumatic event, all of these processes can collapse into a few sentences.

"He started yelling, I froze, and then I left."

That sentence may represent seconds or minutes of extraordinarily complex nervous-system activity.

DBR deliberately slows the sequence.

The therapist may invite attention to the activating material while helping the person maintain awareness of subtle sensations associated with orienting.

Then there may be shock.

Then perhaps a defensive response begins to emerge.

The work is not about forcing the body to perform a movement or manufacturing an emotional release.

Nor is the goal to decide intellectually what the person should have done.

It is to track what arises.

Sometimes the defensive response is obvious.

Sometimes it is remarkably subtle.

A tightening of the hand.

A change in the jaw.

Pressure in the shoulders.

An impulse in the legs.

A sensation of pulling backward.

A nearly imperceptible movement forward.

A sudden stillness.

The percussion section doesn't always arrive with cymbals crashing.

Sometimes the first beat is barely audible.

More Intensity Is Not More Healing

Trauma treatment can sometimes be imagined as a dramatic process.

The person accesses the terrible memory.

Emotion becomes overwhelming.

Something is released.

Healing occurs.

That is an appealing story.

It is also too simple.

A powerful emotional experience is not automatically a therapeutic one.

DBR's emphasis on careful tracking gives us another possibility.

We do not necessarily need the orchestra playing at maximum volume.

We may need enough stability and attention to distinguish what each section is doing.

Where did the response begin?

What followed?

What changed?

What wanted to happen?

What happened next?

The goal is not emotional spectacle.

The goal is precision.

Hearing the Drums Differently

Many people carry shame about their responses to difficult experiences.

They believe they were too emotional.

Too needy.

Too passive.

Too angry.

Too frozen.

Too frightened.

They look backward and judge the person they were from the safety of the present.

Neuroscience cannot tell us that every response was ideal.

Nor should biology be used to excuse every behavior.

We remain responsible for what we do.

But understanding defensive systems can help us separate automatic response tendencies from the moral judgments we place on them afterward.

The body prepared.

The nervous system organized.

Something was trying to happen.

That information can matter.

The Orchestra Continues

We began this series before the first note.

First, something captured attention.

The conductor raised the baton.

Then came the interruption of shock.

The music stopped.

And now defensive systems begin organizing the next movement.

The percussion enters.

There is momentum.

Preparation.

Action.

Inhibition.

Movement toward.

Movement away.

Sometimes both at once.

But there is another instrument we haven't fully listened to yet.

Because some of our strongest reactions do not happen when we are running from danger.

They happen when we are reaching for another human being.

And sometimes the person we are reaching toward is also the person our nervous system has learned to fear.

That is where the music becomes much more complicated.
