The Conductor Raises the Baton

You're sitting in a coffee shop talking with a friend.

Behind you, a glass drops.

Before you have time to think about it, your eyes move. Your head begins to turn. Your attention leaves the conversation.

Then you realize what happened.

Someone dropped a glass.

The whole sequence may take only a fraction of a second.

But notice the order.

You didn't carefully identify the sound, decide that it deserved your attention, and then consciously instruct your eyes and head to investigate.

You oriented.

And then you understood.

This seemingly ordinary moment gives us a window into something fundamental about the nervous system and something particularly important in Deep Brain Reorienting, or DBR.

Before the orchestra plays its first recognizable note, the conductor raises the baton.

Something tells the system:

Pay attention.

We Are Built to Orient

Human beings are constantly surrounded by information.

Sounds.

Movement.

Faces.

Changes in light.

Sensations inside the body.

The feeling that someone has entered the room.

The subtle change in another person's voice.

If we consciously processed every piece of information with equal importance, functioning would be nearly impossible.

Instead, nervous systems continuously select information that deserves attention.

A sudden sound can pull your eyes toward it.

Movement at the edge of your visual field can make you turn your head.

Someone saying your name across a crowded room can interrupt a completely different conversation.

This is part of orienting.

Orienting helps us locate what is happening and determine what deserves further attention.

Most of the time, we barely notice ourselves doing it.

But the brain is doing considerable work behind the scenes.

Meet the Superior Colliculus

Deep within the midbrain sits a structure called the superior colliculus.

Actually, there are two, one on each side.

The superior colliculus participates in integrating sensory information and coordinating orienting movements, particularly movements of the eyes and head toward potentially important stimuli.

If something suddenly moves in your peripheral vision, systems involving the superior colliculus can help direct your gaze toward it.

The point is not that the superior colliculus independently decides whether something is dangerous.

Brains don't work as a collection of tiny executives, each assigned a single psychological job.

The superior colliculus operates as part of interconnected neural systems.

But its role in orienting gives us an important piece of the DBR story.

Because sometimes the nervous system begins responding to something important before we have constructed a conscious explanation for why it matters.

The baton moves before we hear the music.

Now Change the Sound to a Face

A dropped glass is easy to understand.

But imagine something subtler.

You're telling your partner about something that happened during your day.

For a moment, their expression changes.

Maybe their eyes move away.

Maybe their mouth tightens.

Maybe the warmth disappears from their face for half a second.

And something happens inside you.

Your attention locks onto them.

Your eyes search their face.

Your neck becomes tense.

There is a strange sensation somewhere behind your eyes.

Then your stomach drops.

And eventually:

They're upset with me.

Now imagine that you grew up in a home where facial expressions mattered enormously.

Maybe you learned to recognize the difference between an ordinary sigh and that sigh.

The difference between someone setting down a plate and someone setting down a plate.

The difference between a parent walking through the hallway and the particular footsteps that meant you should disappear into your room.

Children can become extraordinary observers of their environments when predicting other people's states matters.

Years later, the environment may be completely different.

But orienting hasn't disappeared.

Your nervous system is still extraordinarily good at noticing change.

The Experience Before the Explanation

This is where people sometimes become frustrated with themselves.

They say:

"I know my partner isn't my father."

"I know my boss isn't going to scream at me."

"I know my friend isn't abandoning me because she didn't text back."

"I know I'm safe."

And intellectually, they may be entirely correct.

But the nervous system doesn't wait for a committee meeting of conscious thought before orienting toward potentially important information.

Something changes.

Attention shifts.

The body begins responding.

Then the conscious mind tries to understand what just happened.

This doesn't mean every reaction is secretly trauma.

It doesn't mean every uncomfortable sensation represents danger.

And it certainly doesn't mean our first interpretation of another person's behavior is always correct.

Sometimes someone really is just tired.

Sometimes a text message really is meaningless.

Sometimes a facial expression has absolutely nothing to do with us.

The nervous system's ability to detect change is useful.

Its interpretation of what that change means is another question.

When Orientation Meets History

Imagine an orchestra preparing to play.

The conductor raises the baton.

Every musician becomes attentive.

But what happens next depends partly on what music they have learned.

Human nervous systems also carry history.

Previous experiences influence what we notice, what we predict, and what our systems have learned deserves attention.

A person who has repeatedly experienced rejection may become exquisitely sensitive to cues associated with disconnection.

Someone who grew up around unpredictable anger may detect tiny changes in tone.

Someone who experienced frightening events may orient rapidly toward particular sounds, movements, places, or bodily sensations.

That doesn't mean the nervous system is defective.

It means it learned.

But learning that once helped someone anticipate what was coming can become painful when similar cues repeatedly activate responses in situations that are different from the original ones.

And sometimes the reaction is so fast that the person experiences only the later part.

Suddenly I was panicking.

Suddenly I wanted to leave.

Suddenly I needed reassurance.

Suddenly I shut down.

DBR becomes curious about the word suddenly.

What happened immediately before suddenly?

Slowing Down the Opening Movement

Many forms of therapy understandably begin with the parts of experience we can describe.

What happened?

What did you think?

What did you feel?

What did you do?

Those are valuable questions.

DBR asks us to become curious about an even earlier part of the sequence.

When the activating material appears, where does attention go?

What happens around the eyes?

What changes in the head or neck?

What sensation appears first?

Is there a moment of shock?

What follows that?

The process can be surprisingly slow.

That is intentional.

Imagine trying to identify the first violin in an orchestra while the entire symphony is playing at full volume.

You might hear the music beautifully, but separating the opening movements would be difficult.

So we slow the composition down.

Not because the story doesn't matter.

Because we are trying to hear something that happened before the story became loud enough to dominate the room.

Why the Eyes Matter

This also begins to explain something that can initially seem unusual about DBR: attention to eye position and orienting tension.

Our eyes don't simply record the world like cameras.

Eye movements are closely connected with systems involved in attention and orienting. Moving the eyes also produces sensory information about the position and activity of the muscles controlling them.

DBR's clinical model places particular importance on this orienting system and uses a specific form of attention to sensations associated with eye position and orienting tension while processing activating material.

Here we need to distinguish neuroscience from clinical theory.

The role of the superior colliculus in orienting and eye movements is established neuroscience.

The specific way DBR uses these systems therapeutically is part of the DBR model and remains an area where further empirical research is important.

Those are not the same claim.

Understanding the difference lets us remain curious without pretending every element of a developing therapy model has already been proven.

When the Conductor Raises the Baton Too Often

There is another way to understand persistent vigilance.

Imagine an orchestra in which the conductor repeatedly raises the baton.

Every time, the musicians prepare.

Again.

Prepare.

Again.

Prepare.

Eventually, even before the music begins, the orchestra may be exhausted.

Some people live with something that feels remarkably similar.

A sound outside.

A facial expression.

A notification on the phone.

A pause in conversation.

A bodily sensation.

A change in someone's tone.

Again and again:

Pay attention.

Something changed.

Find it.

That experience can be exhausting precisely because much of it happens automatically.

People sometimes criticize themselves for being "too sensitive."

But sensitivity is not a complete explanation.

A more interesting question is:

What has the nervous system learned deserves immediate attention?

And then:

What happens after attention arrives?

Because orienting is only the beginning.

The conductor has raised the baton.

The orchestra is ready.

And the next question is what happens when the first note finally lands.

Listening for the Beginning

One of the gifts of studying the nervous system is discovering how much of human experience happens before language.

Before:

I'm scared.

Before:

They're leaving me.

Before:

Something terrible is going to happen.

There may have been a glance.

A movement.

A sound.

A shift of attention.

A tiny muscular response.

An orientation toward something the nervous system determined mattered.

This doesn't make our stories unimportant.

It makes them part of a larger composition.

DBR invites us to listen closely enough that we may begin to notice the opening movement.

Because sometimes the reaction that seems to come from nowhere didn't come from nowhere at all.

We simply entered the concert after the conductor had already raised the baton.
