DBR: When One Section of the Orchestra Plays Too Loudly
The orchestra had been rehearsing for nearly an hour when the conductor finally lowered his baton.
“Again,” he said.
A few musicians shifted in their chairs.
They had been working on the same passage all morning.
It was supposed to begin quietly.
The violins would enter first, almost delicately. The woodwinds would follow. The cellos would join underneath them, and only then would the percussion begin.
At least, that was what the score said.
But every time they reached the passage, the percussionist struck the timpani too soon.
The sound filled the room.
Everything else disappeared beneath it.
The conductor looked toward the back of the orchestra.
“You’re coming in early.”
The percussionist nodded.
They started again.
Violins.
Woodwinds.
Cellos.
And then—
BOOM.
Again.
Too early.
The conductor stopped them.
The percussionist stared down at his mallets.
“I know,” he said quietly.
And he did.
He knew exactly where his entrance belonged.
Knowing wasn't the problem.
His body simply kept preparing to play before the music got there.
Sometimes the Loudest Part Isn't Where the Music Began
This is often how we experience our own nervous systems.
We notice the loud part.
Panic.
Anger.
Shame.
The urge to run.
The urge to shut down.
The desperate need to explain.
The impulse to make someone understand.
The feeling that something terrible is about to happen.
Those reactions can become so intense that they seem like the entire problem.
So naturally, we try to stop them.
Calm down.
Think rationally.
Don't overreact.
You're safe.
Sometimes those strategies help.
But Deep Brain Reorienting asks a different question.
What if the percussion isn't actually where the piece began?
The Orchestra Has an Order
One of the central ideas behind DBR is that responses to threatening or emotionally significant experiences can unfold in a sequence.
Before the full defensive reaction appears, the nervous system first has to detect that something important has happened.
The brain orients.
Attention shifts.
There can be a moment of shock.
Then defensive and emotional responses emerge.
DBR is particularly interested in brainstem systems involved in those earliest moments.
The superior colliculus plays an important role in orienting attention toward significant sensory events, including coordinating movements of the eyes and head.
Something changes in the environment.
The nervous system turns toward it.
Sometimes this happens so quickly that we barely notice it.
Then comes something DBR pays particular attention to:
shock.
That instant when the system registers that something important, unexpected, or threatening is occurring.
Only afterward might the larger defensive orchestra begin to play.
The Percussionist Had Learned Something
The conductor called for a break.
Most of the musicians left the stage, but the percussionist remained.
“You know where the entrance is,” the conductor said.
“Yes.”
“So why are you playing early?”
The percussionist hesitated.
Then he remembered.
Years earlier, he had performed this piece with another orchestra.
That conductor had been unpredictable.
If the percussion section missed an entrance, he would stop the entire orchestra.
Sometimes he shouted.
Sometimes he humiliated the musician who had made the mistake.
So the percussionist had learned to prepare early.
Very early.
He watched constantly.
He anticipated.
He tightened his grip on the mallets several measures before his entrance.
His body learned:
Don't be late.
Eventually, preparation became prediction.
Prediction became reaction.
And reaction became automatic.
The old conductor was gone.
But the percussionist's nervous system didn't know that the way his thinking mind did.
The Brain Predicts
The nervous system isn't simply waiting passively for things to happen.
It is constantly using previous experience to anticipate what might happen next.
That ability is incredibly useful.
If you had to consciously analyze every sound, facial expression, movement, and environmental change before responding, survival would be painfully slow.
The brain learns patterns.
This predicts that.
When this happens, prepare for that.
Traumatic experiences can create powerful predictions.
A certain expression predicts anger.
Distance predicts abandonment.
Criticism predicts humiliation.
Closeness predicts danger.
A mistake predicts punishment.
The prediction may happen before conscious reasoning catches up.
The percussionist hears the beginning of the passage.
His thinking brain knows:
Not yet.
His nervous system says:
Get ready.
And Then Comes the Defensive Response
Once something has been detected and the system has registered threat, other brain systems participate in organizing defensive responses.
One structure relevant to this process is the periaqueductal gray, or PAG, in the midbrain.
The PAG is involved in coordinating survival responses and defensive behaviors.
Depending on the situation and the nervous system's learning history, those responses can include mobilizing to fight or escape, becoming still, or other protective patterns.
In our orchestra, this is when the percussion section becomes impossible to ignore.
The body is no longer simply noticing.
It is preparing to survive.
Heart rate may change.
Muscles tighten.
Breathing shifts.
The urge to move—or not move—can become powerful.
Emotion arrives with force.
By the time we consciously think,
I'm freaking out,
the orchestra may already be several measures into the piece.
DBR Goes Backward Before Going Forward
This is one reason DBR can feel different from therapies that begin primarily with the story or the meaning of an event.
DBR often slows the sequence down.
Instead of starting with the loudest part—
the panic,
the rage,
the shame,
the collapse—
the therapist may help the person find what happened just before it.
Where did your attention go?
What happened around your eyes?
Did you notice anything in your forehead?
Your head?
Your neck?
Was there a moment of shock?
An impulse?
A tiny movement?
The questions can seem almost absurdly specific.
But we are listening for the violin before the timpani.
Because the loudest instrument may not tell us where the music began.
Rehearsing Differently
After the break, the orchestra returned.
The conductor didn't ask them to play the entire passage.
Instead, they stopped several measures before the percussion entrance.
“Don't play,” he told the percussionist.
“Just notice when you start preparing.”
The orchestra began.
The percussionist listened.
Violins.
His hands were relaxed.
Woodwinds.
Then something happened.
His shoulders tightened.
His fingers closed around the mallets.
There.
Several measures before his entrance.
He had never noticed it before.
They tried again.
And again.
Not forcing the response away.
Not criticizing it.
Simply slowing the sequence enough to hear where it began.
The Quiet Parts Matter
Trauma treatment often focuses on experiences that feel enormous.
And understandably so.
But sometimes the path into those experiences runs through something very small.
A movement of the eyes.
A tightening of the neck.
A subtle orienting response.
A moment of shock.
A defensive impulse beginning to form.
These are not dramatic experiences.
They are the quiet measures before the crescendo.
DBR works carefully with this territory because the nervous system may have learned its protective sequence there.
And when that sequence can be processed rather than simply repeated, something begins to change.
The Percussionist Waits
They reached the passage again.
The violins entered.
Then the woodwinds.
The percussionist felt his shoulders tighten.
He noticed his hands preparing.
The familiar prediction appeared.
Now.
But the conductor hadn't raised his baton toward him.
So he waited.
One measure.
Then another.
The cellos entered.
And finally the conductor looked toward the back of the orchestra.
The baton moved.
The percussionist played.
The timpani filled the concert hall.
Loud.
Powerful.
Exactly where it belonged.
The conductor smiled and continued.
Because the goal had never been to silence the percussion.
An orchestra needs percussion.
A nervous system needs defensive responses too.
Fear has a purpose.
Anger has a purpose.
The impulse to escape has a purpose.
Even freezing can be an extraordinarily sophisticated survival response.
Healing does not mean removing those instruments.
It means that the nervous system no longer has to play them simply because an old piece of music has begun.
Sometimes the violins can just be violins.
A pause can just be a pause.
A facial expression can just be a facial expression.
And the percussionist can sit quietly at the back of the orchestra, mallets resting in his hands, knowing that if the music truly calls for him, he will be ready.
